Before completing the registration form on this page, Best Companies Group needs you to agree to the Best Places to Work in Chicago program terms and conditions. The form will not submit until all mandatory fields are filled out completely and accurately.

I understand that we are responsible for paying for any additional services ordered (customization, translation fees, additional reporting, etc.). If we withdraw after the registration deadline $125 withdrawal fee to remove our organization from the process.

Tell us about your people!

Primary Contact

The single point of contact who steers your participation from start to finish—receiving every email, call, survey link, report, or mailing from us and routing each item to the right people inside your organization.

Is this contact an employee of the company?

Secondary Contact

The designated stand-in for the Primary Contact, to only be used when the Primary is unavailable: vacation, leave, or role change. Choose someone who would be ready to step in.

HR Contact

HR colleague we can consult for survey data or workforce policies-related questions. All portals, questionnaires, and results still go to your Primary Contact; we’ll contact this HR person only for those HR-specific follow-ups.

IT Contact

A technical point person in addition to Primary Contact to troubleshoot survey-delivery issues, allowlist domains, resolve spam or filter blocks, and handle system questions that keep our surveys and links from flowing smoothly.

Number of Employees: Tell us how many employees you have working in Chicago.

If you employ more than 250 people, we will survey a random sample of your employees. Please enter the total number of employees (See Eligibility) working at this organization. Please do not include temporary, seasonal or per diem employees, PRN, 1099 employees/independent contractors, volunteers, interns, outside consultants or staffing employees placed by other organizations. (See Eligibility)

*To ensure the credibility of the information, companies with 15-24 employees must have an 60% (or better) response rate on the employee survey to be considered for the list. **Employees are randomly selected. Best Companies Group offers the option to survey more employees than the random sample calculated in the above chart, and/or include additional employees in the survey process who work in locations outside of the program area.
